ALL FOLLOW UPS ARE VIA PHONE ARIZONA TIME UNLESS SCHEDULED OTHERWISE

HAVE ALL TEST/LAB RESULTS AT FIRST APPOINTMENT Most recent results and all current medications & supplements. If you haven’t had labs in the last year, please contact me.

ALL LABS Should be collected fasting, that is NOTHING but water for 8-12h before draw, no meds, supplements, food, & be sure to schedule your appointment with on-line scheduler to review labs.

ADDITIONAL INFO TO COMPLETE:

Payment is expected at time of service.

Cancellations require 24 hour notice.

Documentation provided w/ necessary medical codes to submit to insurance company for reimbursement/utilize HSA funds.

Reimbursement cannot be guaranteed & deductibles may apply, including laboratory testing & imaging; contact insurance company for more information.

Contraception Prescriptions: $50

For any patient, new or established, 13+ years of age, telemedicine for prescription for contraception or emergency contraception
