Payment is expected at time of service.

Documentation (Superbill”) is provided with necessary medical codes for “out of network” services” to submit to insurance for reimbursement/utilize HSA funds if you choose.

Reimbursement cannot be guaranteed & deductibles may apply; it is recommended you contact your insurance company for more info.

Labs, imaging, and medications are covered based on the benefits of your specific insurance plan.

Sliding scale may be an option for those in need.

New Patients: $200 (Military discount 25% off); TELEMEDICINE will be the default for new patients, unless otherwise requested. Here is the link

Return Patients $90; via phone unless otherwise arranged

Contraception Prescriptions - $50.

For any patient, new or established, 13+ years of age, telemedicine for prescription for contraception or emergency contraception.
